This completed application and agreement form confirms agreement to the Conditions of Filming on Champagne and Aishihik First Nations (CAFN) Traditional Territory.

CONDITIONS OF FILMING:

  • All representations of CAFN people and CAFN culture must be carried out in a respectful manner, including an intent to fully share the intended film footage and storyline as it will be portrayed in any media, prior to actual publication release or broadcasting.
  • All interviews require a film interview release form signed by both the interviewee and a representative of the Film Company, to ensure the interviewees have the opportunity for provide informed con‐ sent, prior to the carrying out of interview and filming.
  • The film interview release form must explain in sufficient detail the purpose of the interviews, the in‐ tended use of the documentary footage, the method of acknowledgement for the individuals, and any other details with respect to how the individual can revoke such permission in the future.
  • The Film Company agrees to provide copies of all interview release forms to CAFN, so CAFN can monitor the use of these interviews in the future.
  • The Film Company will provide CAFN with a list of footage obtained at each of the filmed events for review by CAFN. CAFN may request copies of the footage in disc format and understands there may be a nominal fee for this service.
  • The CAFN government has a primary interest in the protection of traditional knowledge for all of the Champagne and Aishihik First Nations. If the designated CAFN staff member who reviews the draft documentary footage determines that the content includes sensitive traditional knowledge, the undersigned will agree to not include such identified documentation in any published or broadcast media.
  • The Film Company will provide CAFN with copies of the edited film project.
  • The Film Company agrees not to use any of the footage for any other purpose than agreed to by CAFN. This means the Film Company must be in possession and control of the documentary information at all times. Any transmission, copying, re‐location, or usage of the documentary resources by any other Party than those identified in this agreement, requires consultation and consent of the Champagne and Aishihik First Nations.
  • CAFN must be provided a request from the Film Company for any intent to release any of the documentary resources from this agreement.
  • The Film Company acknowledges that all information shared by CAFN citizens and/ or representatives of the CAFN government, remains the intellectual property of Champagne and Aishihik First Nations, and shall be credited as such.
