Job Summary:
Reporting to the Healthy Living Manager, this position is responsible for providing comprehensive wellness support and case management to CAFN Elders and their families in the areas of community care, resource access, NIHB navigation, medical systems support, mental wellness, addictions and trauma, including outreach prevention. This position also serves as a liaison and support between other Governments or agencies including NIHB, First Nations Health, Yukon Hospital Corporation, Regional health centers, Continuing Care, Adult Protection, Yukon Housing and Mental Wellness & Substance Abuse Services. This position will assist with coverage of the Community Care Coordinator position when required, including oversight of the Elders Fuel Subsidy and Community Care workers.

Education and Experience:
Bachelor of Social Work degree, or completion of a 2-year diploma in Social Work, Nursing, Home and Community Care, or/and minimum of 5 years of experience in home and community care, social work, long-term care or related field with several years’ experience in case management, and/or home and community care, preferably in a First Nations environment or an equivalent may be considered.

  • Conditions of Employment:
  • Valid Yukon Class 5 Driver’s License; Class 4 Driver’s Licence would be an asset
  • Clear Criminal Record Check with Vulnerable Sector Check
  • Willing and able to travel to other CAFN communities weekly
  • Willingness to work evenings and/or weekends, as required
  • Extended workdays may be required to accomplish tasks and meet workload demands during peak periods, as directed
